Cambridge Care Company have trained assessors to be able to offer a complete assessment and monitoring care package in an emergency and/ or short notice situation.
This can be very useful in hospital admission prevention or part of discharge from hospital planning.
We will visit you at home and complete the initial assessments and provide a trained member of staff to monitor and care for the person in need for 24-48 hours, once this period is over we will then provide our full assessment and discuss and implement a care package to meet your ongoing needs.
Cambridge Care Company firmly believe in enabling independence and the assessments tools we use are person centred and tailored to the individual. We also offer advise and information for other services which could benefit you from assistive technology to community services in your area.
